In printing the Liability Waiver form:

1) Print page 1 from the Adobe Reader print box by changing the ' TO PAGE ' number to 1.
2) After the first page prints, remove, reverse and reload the page, click the print symbol and change the ' PAGES FROM ' number to 2.
3) When printing multiple pages, the same changes listed above as well as the ' COPIES ' box must be changed.

NOTE: It is important from a legal standpoint that the document be printed as a two-sided page.
